These are required for ALL events. Event Chairperson is
responsible for completing all necessary forms as required. Note,
there are several forms available at the main link, including
parental consent forms and claim forms, as well as FAQs and
other information. Members, especially Event
Chairpersons, are encouraged to review all the information
provided, not just the general waiver. The link goes to the NCCC
Insurance page, which contains all necessary forms.
Reminder: Each member must sign waivers for themselves -
spouses/significant others/friends CANNOT validly sign waivers
for another person!
